He is survived by: his father Vern Perkins; his wife Kelly Perkins (Worthen); his daughters, Whitney Perkins (Matt Rakestraw) and Lauren Perkins; his stepchildren, Brayden and Kayla Tosi; his grandchildren, Braden, Adrien and Reina; his sister Lisa Esposito (Tony); his parents-in-law, Tom Worthen and Judy Worthen; his sister-in-law Tracey Castle; his nieces and nephews, Ashley Bussey (Matt), Thomas Piatt (Lottie), Madison Piatt, Jonathan Esposito (Dana), Jaycie Floyd (Dustin) and Anthony Esposito (Kaylee); and his friends, Joe Beers, Billy Fultz and Kathy Frantz-Cogdill. He is also survived by 12 great nieces and great nephews.
