He is survived by: his mother Mylie Efferin (Self); his wife Tina Self (Leach); his stepfather Michael Efferin; his children, Jacob Daniel Lee Self and Rebeca Lynn Self; his stepdaughter Nicole Ackerman; and his brother Dwayne Self Jr..
