He was predeceased by: his daughter Cassie Jones. He is survived by: his parents, Gary L. Cook Sr. and Beverly Stiles (Hill); his sons, Paul Cook (Allison) of Portsmouth, Jacob Cook of South Webster and Kayden Cook of Portsmouth; and his siblings, Gary Cook Jr. (Tonda) of Wheelersburg, Howard Stiles of South Webster and Nikki Maloney (Jeremy) of South Webster. He is also survived by one grandchild and several aunts, uncles and cousins.
