He was predeceased by: his parents, John Slack and Sarah Slack (Johnson); his wife Eunice Slack (Jarvis); and his brothers, Melford, James, Lawrence and Leroy.
He is survived by: his daughter Melody Slack of South Webster; his sisters, Delsie King (Tom) of Lucasville and Dearie Godfrey (Dwaine) of Lucasville; and his grandchildren, Eric Slack (Deamie) and Megan Freedman (Steven). He is also survived by many great-grandchildren and great-great-grandchildren.
