He was predeceased by: his parents, James and Jane. He is survived by: his wife Carol; his stepchildren, Kristen Bergman (Brian Welms), Sarah DeMoss (Aaron), Matthew Bergman and Hannah Bergman; his siblings, Dave (Carrie), Tim (Marianne) and Barbara Flate (Steve); and his pet Rocky.
In keeping with Dan's wish to help others, he has donated his body to the School of Medicine at Case Western Reserve University. Contributions in memory of Dan suggested to Jennings at Brecksville, 8736 Brecksville Rd., Brecksville, OH 44141.
