He was predeceased by: his parents, William Cook and Verneda Cook (Baker); and his siblings, Jean McCoy and Thomas Cook. He is survived by: his wife Linda Louise Fogt; his children, William Oliver Cook (Kelly) of Conover and Lynn Lee Cook of Sidney; and his siblings, Dennis Cook of Sidney and DeAnna Slife of Sidney. He is also survived by four grandchildren.
