He was predeceased by: his son Alex Garner Van Hook; his father Carl Garner Van Hook; and his brothers, Joel Van Hook and Tim Van Hook. He is survived by: his mother Mary Van Hook (Phelps) of Sidney; his children, Eric Tyler Van Hook (Anna) of Sidney and Asley Neal (Tomas) of Piqua; his grandchildren, Hailey Nicole Van Hook, Alyvia Dunn, Quentin Neal, Cleveland Neal, Lucy Neal, Jet Neal, Layla Atwood and Briar Atwood; his siblings, Bike Van Hook (Melinda) of Sidney, Steve Van Hook (Tina) of Sidney and Richard Van Hook of Hillsboro, OH; his mother-in-law Marge Davis; and his brothers-in-law, Terry Davis and Horbin Brown.
