She was predeceased by: her siblings, Martha Cotterman, Harry Clark Sr., Jo Ann Lyme, Sue Pequinot (Andy), Henry Bynum and George Bynum (Ruth); her sons, Robert (Bim) and David Wade (Scott); and her sister-in-law Jean Clark.
She is survived by: her parents, Theodore Clark and Nell Clark (Chaney); her spouse Warren Cook; her children, Deborah Beaver (Roger) of West Milton, Kelly Cornett (Dave) of Sidney, Dan Wade (Leisa) of Sidney, Dawn Cox (Alvin) of Sidney, Tim Cook and Rick Cook; her siblings, James Clark of Sidney and Sarah Pequinot (Andy) of Berry; her brother-in-law Ron Cotterman; and her grandchildren, Rickey Driskell, Bo Driskell, Brian Driskell, Jeremy Wade, Christopher Wade, Patrick Wade, Ashley Cantrell, Jordan Wade, Josh Bodenmiller, Jarvis Bodenmiller, Danielle Wade, Emmi Wade, Tony Donahue, Tabby Donahue, Heidi Cox, Erica Cook, Hilary Cook and Jason Cox. She is also survived by numerous nieces and nephews; several great-grandchildren; and several great-great-grandchildren.
In lieu of flowers, donations may be made to Alzheimer's Association in Dorothy's memory.
