He is survived by: his daughter Angelina Fourhman (Ryan) of San Diego; his girlfriend Charlotte Lacey of Greenwich; his parents, Horace Cook and Wanda Cook; and his siblings, Angela Walton (Paul), Karla Street (Brian), Sarah Vartanian (Kent), Jesse Cook (Stephanie) and Amber McDaniel (Andrew). He is also survived by numerous nieces and nephews.
