He was predeceased by: his wife Jean A. Dziak; his parents, Charles Dziak and Susan Dziak; and his siblings, Joseph, Charles Jr., Paul, John Dziak, Anna Zadorozny, Susan Lehman, Pauline Kudrin, Mary Mauro, Agnes Dziak and Margaret Haller.
He is survived by: his children, Andrew J. Dziak (Mary Jo) of Wakeman, Timothy Dziak of Sheffield Village, Columbus, Christine Dziak Fenik (Theodore) of Vermilion, Douglas Dziak (Leigh Anne Mosby) of Arlington and Virginia; his grandchildren, Rachael, Patrick Avery, Danielle, Andrew Dziak, Jennifer and Nicholas Fenik; and his sister Katherine "Kay" Brest of Phoenix, AZ. He is also survived by many nieces and nephew.
