He was predeceased by: his parents, Theodore Falkenberg and Christine Falkenberg (Loesch); and his wife Marilyn Falkenberg (Kurtz). He is survived by: his children, Kimberly (Tom) and Hudson; his grandchildren, Kaitlyn, Gavin, Madison, Ryan, Brandon and Ayden Falkenberg; and his sisters, Teddy West (late Eldon) of Freedom Twp and Lorelei Lopick (Rodney) of Canton. He is also survived by 14 nieces and nephews.

The family would like to thank all the staff at Maplewood for the wonderful care they took of Otto over the past 4 years and requests that in lieu of flowers, contributions may be made to: Maplewood At Twinsburg, 2463 Sussex Blvd., Twinsburg, Ohio 44087.
