He was predeceased by: his parents, Thomas Melvin Mack II and Rose Etta Cerett Kozak; and his brothers, Steven Mack and Kenneth Mack. He is survived by: his wife Jodi Lynn Mack; his children, Thomas "TJ" Melvin Mack IV (Savannah Re Mack) and Erika Elaine Mack; and his grandchild Gianna Elaine Mack.
He family asks that memorial contributions may be made in his name to Scio American Legion Post 482, 38175 Crimm Rd, Scio, Ohio 43988.
