He was predeceased by: his parents, Theodore Fazzari and Erma Fazzari (Hill); and his wife Ethel L. Fazzari (Terembes). He is survived by: his children, Tanja Fazzari (James O'Dell) of Columbus, Todd Fazzari (Sheri) of Dayton and Tara Fazzari (Joseph Pepper, II) of Seattle, WA; his siblings, Anthony Allen Fazzari of Millsboro, PA and Elizabeth Atalski of Fort Mills, SC; and his grandchildren, Connor Fazzari, Jenna Fazzari and Tre Pepper.
