He was predeceased by: his parents, Harry Sample, and Opal Sample, Sr. (Bowman); his wife Jean Adele Juby; and his siblings, Rosemary Coleman, Eileen Frye, Harry Sample, Jr. and Knolan Sample.
He is survived by: his children, Marla Laney (Thomas) of Sandusky and Mark Sample (Karin) of Orrstown, PA; and his grandchildren, Steven Chudy, Gwen Cromer, Matthew Sample and Tim Sample. He is also survived by twelve great-grandchildren; three great-great-grandchildren.
