He was predeceased by: his aunt Rita Blossom. He is survived by: his son Joshua Andrew Lee Cozad; and his siblings, Dustin Cozad (Brandi) of Van Wert and Brittany Cozad of Poneto, IN. He is also survived by 3 nieces; 2 nephews and 1 great niece.
