He was predeceased by: his parents, Suzanna Hutira Slavik and Emil Slavik Sr.; and his brother Edward Slavik. He is survived by: his wife Delores Jean "D. J." Niekro Slavik; his children, Emil A. Slavik (Karen), Michael Slavik (Carla), Diane Thornburg (Dick) and Dave Slavik (Leta); his brother John Slavik (Charlotte); his grandchildren, Katie Krcal (Craig), Christopher Slavik (Morganne), Evan Slavik (Natalie), Ryan Slavik (Haylee), Alex Thornburg (Cathy), Andy Thornburg (Anna), Drew Slavik (Sarah) and Trent Slavik; and his great grandchild June Thornburg. He is also survived by many nieces and nephews.
