He was predeceased by: his parents, Rethel Pack and Myrtle Francis Pack Sr. (Bolt); and his siblings, William Pack, John Pack, Lora Mae Hoover (Pack) and Mary Arvin (Pack).
He is survived by: his girlfriend Ada "Bunny" Moore; his children, Dale Moore Jr. and Yvette Lyons (Anthony); his grandchildren, Jessica Horner, Hannah Hawke and Anthony Lyons Jr.; and his great grandchildren, AJ Lyons III, Raleigh Welsh, Aerilyn Lyons, Lucie Horner, Donny Pack of Virginia, Rick Pack of Maryland and Tom Pack of California.
