He was predeceased by: his parents, Glenn Campbell and Phyllis Campbell (Caudill). He is survived by: his wife Linda Campbell (Fornash); his children, Tammarra Lynn Campbell, John Jacob Campbell and Joshua Abraham Campbell; his siblings, Joe Campbell (Bob Reis) and Teresa; his nephew Ross; and his nieces, Alexandra and Andrea Sferra. He is also survived by other loving relatives and friends.
