He was predeceased by: his parents, Richard J. and Rose Marie Perkowski (Ulatowski); and his siblings, Mary Witkowski, Richard J. Perkowski and James Resk.
He is survived by: his wife Darlene Perkowski (Hlawati); his children, Barbara Perkowski (Joseph Goodale) of Pittsburgh and Timothy Perkowski, Jr of Rock Creek; his grandchildren, Cameron Goodale and Levi Perkarski; his siblings, Arthur Resk of Ashtabula and Michelle Kuchar (Scott) of Rock Creek; and his brother-in-law Daniel Witkowski.
