He was predeceased by: his grandmother Sue Coffman. He is survived by: his parents, James Wells and Peggy Wells (Fogle); his children, Adrinne Snyder, Jakotah Snyder (Summear) and Jerry Swihart; and his siblings, Jason Wells (Jessica) of Adam Wells, Tony Snyder of Adam Wells and Angel Wells.

In lieu of flowers memorial contributions in his honor may be made to Nationwide Children's Hospital, 700 Children's Drive, Columbus, Ohio 43205.
