He was predeceased by: his father Cecil Smith; his wife Eva Louise Smith (Spencer); his grandmother Osborne; and his sister Mary Spencer. He is survived by: his children, Perry Smith Jr. (Donna) of Lucasville, Janey Frazier (Randy) of Rosemount and Sheila Winn (Steve) of Minford; his son-in-law Donald Osborne; and his siblings, Kenny Tackett, Danny Smith and Betty Jo Smith. He is also survived by six grandchildren and four great grandchildren.
