She was predeceased by: her parents, Thad Caldwell and Julia Caldwell (Nelson); her husband Charles L. Cook; her daughter Melanie; and her siblings, Charles Caldwell, Vera Caldwell, Mindy Mollett and Cora Marshall.
She is survived by: her daughters, Shirlene McCoslin of Portsmouth and Christine Sanders (James) of Portsmouth; her grandchildren, J. W. Sanders (Jaylynn) of Wheelersburg and Amy Howard (Michael) of Sciotoville; her great great grandchild Renesmee Sanders; and her siblings, Jim Caldwell (Brenda) and JoAnn Grashel (William) of Wheelersburg.
