He was predeceased by: his parents, Andrew Kozlok and Margaret Irene Kozlok (Truex); his brother Jimmy Kozlok; his grandson Mason Andrew Kozlok; and his nephews, Jonathon A. Kozlok and Scott W. Kozlok.
He is survived by: his children, Sean Kozlok (Jill) of Port Clinton and Kristy Greener (Andy) of Oak Harbor; his grandchildren, Ayden Kozlok, Brynn Kozlok and Keegan Hunsinger; and his brother Teddy Kozlok (Tanya).
