He was predeceased by: his father Harold J. Kubasek Sr.; his stepfather Orlyn Stewart; and his brother-in-law Danny Lakin. He is survived by: his wife Pamela S. Kubasek (Lakin); his mother Donna J. Stewart (Hummel); his siblings, Diane Keegan (Thomas Keegan), Debra Ballard (Kubasek) and Harold Kubasek Jr.; his sisters-in-law, Jennifer Orosz (Lakin) and Patricia Lakin; his niece Danielle Lakin; his nephew Benjamin Lakin (Quinton Orosz); and his great-nephew Lucas Russell.
In lieu of flowers please give generously "in Memory of David P. Kubasek" to The Wounded Warriors Project, a charity that was near to Dave's heart or Wreaths Across America.
