It is with great sadness that we announce the death of Dale Middaugh of Polk, Ohio, born in Lodi, Ohio, who passed away on April 30, 2021, at the age of 78, leaving to mourn family and friends. He was predeceased by: his parents, Melvin Middaugh and Opal Dravenstott Middaugh; and his sister Shirley Middaugh. He is survived by: his wife Cheryl Trapp Middaugh; his children, Aaron Middaugh of Polk, Andy Middaugh (Veronica) of Polk and Amy Sampson (Randy) of Ashland; his stepsons, Stephen Lynn Jefferies of Loudonville and Steven Carl Smith (Donna) of Henderson; and his grandchildren, Paige, Kallie, Masilyn, Mia Sampson, Mayson, Sydnee, Gracie Lynn Jefferies and Brooke.
