He was predeceased by: his parents, Jeremiah J. Sisk and Betty H Sisk (Clettenberg); and his son Jonathan Sisk. He is survived by: his wife JoAnne Sisk (Gates); his brother Terrence Sisk; and his children, Tammy, Jeffery (Carrie), Jerilyn, Jerry Jr. (Cheri) and Jason (Nick). He is also survived by many grandchildren and great-grandchildren.
