He was predeceased by: his parents, Frank Polachek and Margaret M. Polachek (Hogue); and his wife Nancy E. Polachek (Weiss). He is survived by: his children, Greg Polachek (Mandy) of Willard and Sandi Weltlin (Aaron) of Plymouth; his grandchildren, Brad Polachek (Kayla) of Willard, Mariah Polachek of Willard, Nathan Weltlin of Plymouth and Grant Weltlin of Kent; his great grandchildren, Owen, Riley, Tyler and Jaxson; and his sister Mary Jayne Hall (Aubrey) of Willard.
In lieu of flowers, memorial contributions may be made to the Plymouth Athletic Boosters or Willard High School Community Veteran Fund.
