He was predeceased by: his sister Beverly; and his brother-in-law Mel Eisaman. He is survived by: his wife Paula Suzette Slade; his son Anthony Allen; his daughter-in-law Chandra Slade of Piqua; his adoptive daughter Paula Suzanne Adams of Sidney; his stepsons, Jason Brunson of Piqua and Daniel (Sherry Brunson) of New Carlisle; and his siblings, Larry Slade of Sacramento, CA, Dorothy Hardwick of Dayton and Marilyn Johnson of Arkansas. He is also survived by nieces, nephews, other relatives and friends.
