He was predeceased by: his parents, John Hawkins and Lady Cutler. He is survived by: his wife Bonnie M. Gambill May; and his children, Herbert Hawkins Jr. of Piqua, Randy Hawkins (Darla) of Piqua, Connie Barnhart (Tim) of Sidney and Pamela Kimbrell (Robert) of Piqua. He is also survived by several grandchildren; and several great grandchildren.
