He was predeceased by: his parents, Chester A. and Zola Collins, Sr. (Blair); his brothers, Chester A. Collins, Jr. and Dallas Collins; and his fiancée Bonnie Dawkins.
He is survived by: his siblings, Connie Gurski of St. Paris and Jim Collins of Piqua. He is also survived by many other nieces and nephews.
