He was predeceased by: his parents, James Antoniak and Louise Antoniak; and his brother William Antoniak. He is survived by: his wife Kathy; and his brothers, James Antoniak (Barb) of Columbus, Ted Antoniak and Tim Antoniak. He is also survived by sisters.

In lieu of flowers, contributions can be made in Mark's name to First Tee of Central Ohio.
