He was predeceased by: his parents, Gaylord Stevens and Mary Stevens (Whitt); and his sister Dreama Thomas. He is survived by: his wife Patricia; his children, Audrey, Jack Jr. and Tim; his grandchildren, Matthew, Austin and Ashlea; his great grandchildren, Gabby and Gracie; and his siblings, Dee Stevens, Dorothy Swindall and Pauline Smith. He is also survived by nieces and nephews.
