She is survived by: her sons, Dennis Smazenko (Christine) of Fair Oaks, Allen Smazenko of Van Wert, Gary Smazenko (Betty) of Antwerp and Andrew Smazenko (Theresa-Terri) of Hicksville.

In lieu of flowers Phyllis's family request memorial contributions to the American Diabetes Foundation and Research.
