He is survived by: his close friend Gladys Sue Johnson of Patriot, Ohio; his daughter Jonnette Hazen (Jeff) of Fort Worth, Texas; his grandchildren, Payton Hazen and Colton Hazen; his great grandchild Emerli Hazen; and his stepchildren, Keith Day (Cheryl) and Roberta Johnson.
