He was predeceased by: his parents, William Cook and Mary Cook (Kelder). He is survived by: his wife Bonnie Cook (Boyd Wallace); his sister Eleanor Yost (Cook); his children, William Cook (AnnMarie), Rebecca Hockstok (Jason), Christopher Wallace (Shannon) and Bryan Wallace; and his grandchildren, Jake Cook, Emily Cook, Allison Cook, Amber Hockstok, Ashley Hockstok, AJ Hockstok, Maxwell Wallace, Madi Wallace, Owen Wallace and Nolen Wallace. He is also survived by many nieces, nephews, and extended family.
