He was predeceased by: his parents, Walter "Pete" Smith and Amy Smith (Grandstaff); and his sisters, Cheri Ladig and Sheila Wojiechowski. He is survived by: his daughter Dr. Lisa Holtsclaw (Jeffery) of Fort Wayne, IN; his grandchildren, Ross Holtsclaw of Bethlehem, PA, Heidi Holtsclaw of Fort Wayne, IN and Noah Holtsclaw of Fort Wayne, IN; his daughter Angela Janssens (Jack) of Fort Wayne, IN; and his sisters, Elaine Rhonehouse (Don) of Payne, OH and Anita Suman (Jack) of Defiance, OH. He is also survived by her children and grandchildren.
