He was predeceased by: his father Otis Richard; and his former wife Dawn M. Turner. He is survived by: his children, Tracy Hettinger (Joe) of Johnstown and Scott Cottle (Tammy) of Leipsic; and his grandchildren, Kortney Salinas (DeLee) of Ada, Mark Hettinger of Johnstown and Jason Hettinger of Johnstown.

Memorial donations may be made to Ottawa Food Pantry or Putnam County Council on Aging.
