He was predeceased by: his wife Ruth Helen Puthoff; his grandson Adam; his parents, Albert Puthoff and Margaret Puthoff; and his siblings, Ralph and Mary.

He is survived by: his children, Steven Puthoff (Lanita), Michael Puthoff (Deborah), Jennifer Setser (Richard), Paul Puthoff (Julie) and Mary Beth Calendine; his grandchildren, Andrew, Samantha, Alixandra, Miranda, Christopher, Nathaniel, Jacob, Joshua, Connor, Molly and Nicholas; and his siblings, Robert "Duke", Suzanne (Rod), Richard (Stephanie), Joanne, Roger (Debbie), Victoria (Michael), Raymond (Linda) and Gregory (Tammy).

In lieu of flowers, memorial donations may be made to the St. Louis Church or the St. Louis School.
