She was predeceased by: her father John W. Walentik; and her brother John J. Walentik. She is survived by: her mother Patricia L. Walentik (Workman) of Orrville; her fiancé William Leavens of Orrville; her children, Danica Thomas (Scot) of Orrville, Ben Carr (Tara) of Shreve and Brent Carr (Amy) of Apple Creek; her grandchildren, Mackenzie, Blake, Alicea, Olivia, Courtland, Asher, Braylon, Lydia, Kyle and Evelyn; her sisters, Ruth Johnston (Jay) of Orrville and Mia Bryant of Shreve; and her sister-in-law Pam A. Walentik of Orrville. She is also survived by many nieces, nephews and cousins.
