She was predeceased by: her parents, Theodore Keefer and Rosamond Keefer (Gilbert); her son Dane F. Worthington; and her sister Korinne Pasqualone. She is survived by: her husband Glenn; her children, Gail L. Keaton, Dale L. Tebbe (James) and Dean P. Worthington (Noi); her grandchildren, Michael J. Haynes, James M. Tebbe, Madison G. Tebbe and Alexandra P. Worthington; her great grandchildren, Trenton and Michael "Will"; and her siblings, Gilbert "Butch" Keefer (Gail) and JaNell Christian. She is also survived by husband, helpmate, and partner to her, but became a beloved father to her children and later a doting grandfather and great grandfather.
In lieu of flowers, a donation may be made to The New Life Assembly of God in Oregon, OH or a memorial gift in Phyllis' memory can be made to the Hospice of Northwest Ohio.
