She was predeceased by: her parents, Victor Czajkowski and Lottie Czajkowski (Rybka) of Toledo, Ohio.; and her husband David F. Forrest. She is survived by: her son Jacob Forrest (Dawn); her grandchildren, Kiley Ann, Kane David of Millbury, Seth Forrest (Jennifer), Ava Marie and Carter Thomas of West Milton; her sister Bernadette Pugh; and her niece Chrissy Mackey (Keith).
