She was predeceased by: her parents, Jeanette Swartz and Donald Swartz; her brother Donald Jr.; her brother-in-law Robert Zuchowski; and her grandparents, Lawrence, Cecil Swartz, Stephen and Mary Koles.
She is survived by: her children, Tiffanny Adams (Eric), Courtney Woody (Joe) and Peter Martinez; her grandchildren, Joey (Felicity), Jimmy (Tori), Jacob, Anthony, Jesalynn, Amelia, Amira, Greyson and Peter; her great grandchildren, Jaxson and Emmy Jo; and her sisters, Debra Zuchowski and Tracy Gribble (Derek). She is also survived by aunts, nieces, nephews, and cousins.
Memorial donations in Diana's name may be made to Hospice of Northwest Ohio or to her family for final expenses.
