He was predeceased by: his parents, John Folkerth and Carolyn Folkerth (Wood). He is survived by: his children, Ally and Edward; and his brothers, John (Karen) and Bob (Robin). He is also survived by several nieces and nephews.

“I was sad to hear about Dave Folkerth. He was a very kind and gentle person and a friend to everyone he ever knew. I remember growing up with him in our neighborhood, running around outdoors and playing poker in his basement with all the boys in the neighborhood. Dave liked basketball and was the leader in organizing several intermural teams at Oakwood junior high and high school. All of them had really ridiculous names consistent with Dave’s good nature and sense of humor. A unique man.”
