He was predeceased by: his parents, John Asa Midkiff of West Virginia and Rhoda Midkiff (Cassle) of Oak Hill, Ohio.; and his daughter Jennifer Logan Bishop. He is survived by: his wife Marilyn "Sue" Midkiff (Phillips); his daughters, Tina Midkiff and Crystal Glenn; his grandsons, Jonathan "Zachary" Midkiff, Dylan Shively, Garrett Shively, Kaleb Midkiff and Kyle Midkiff; his siblings, Sandra Crabtree (Jack), Nancy Crabtree, Terry Midkiff (Vicki) and Ronnie Midkiff; his siblings-in-law, Pat Carothers (Chip), Ron Phillips and Ken Phillips; and his close friends, Garry Salmons, George Snyder, Shane "Bubba" Hammer, Fred Bishop, Kelsey Hilderbrant, Kenny Wallace, Kaitlyn Bell, Megan and Lunsford.
