He was predeceased by: his parents, Izarah Sullivan and Christine; and his sister-in-law Jan. He is survived by: his wife Wilma Sullivan (Roberts); his daughters, Michelle Worley (John), Amy Hoffard (Andy) and Christina Herrick; his grandchildren, John Carl, Emily, Jenna, Andrew, Adam, Anna, Payton, Everly and Elena; his great grandchildren, Colton, Cooper, Andrew, Jaxon, Wade and Gracie; and his siblings, Nancy Kemme (Steve), Mary Feldman (John), Mona Sirk (Don), Robert (Bonnie), Garrett (Cheryl), Allen and Paul.
