He was predeceased by: his parents, Victor Perkins and Vera Perkins (Metz); and his siblings, Richard Perkins, David Perkins and Joyce Carter.
He is survived by: his wife Nancy; his daughters, Julie Collins (Caine) and Amy Foltz (Scott); his granddaughters, Emma, Abigail Collins, Jenna, Kylie, Lainey and Macie Foltz; and his sisters-in-law, Glenna Perkins and Dorothy Perkins.
