He is survived by: his wife Sarah; his children, Jason Adam Qualkinbush (Lea) of Munroe Falls, Jamie Lee Maassen (Matt) of Elyria and Jodie Ann Gregg (Lyle) of North Ridgeville; his grandchildren, Ken, Andrew, Ashlie and Dakota; his brother Stephen Ray Qualkinbush (Jeannie) of Wintson-Salem, NC.; and his stepsister Phyllis Garrison of Indianapolis.
