He was predeceased by: his parents, Harry E. Slack and Hattie Slack (Johnson); his wife Ruth Ann; his siblings, Helen Hollback, Wanda Knight, Ronald Slack, Jeanne Stiles and Thelma Hale; and his stepmother Ruth Slack.
He is survived by: his children, Karen Endres (Richard), Kathy Slack, Glenn Slack (Jeanie) and Nancy Smith (Dan); his siblings, Elnora Blanton, Marlene Slack and Paul Slack; his grandchildren, Ryan Endres (Jackie), Renee Endres, Rachel Endres, Andrew Slack, Rachel Morgan (Ryan), Luke Smith, Seth Smith, Shane Smith (Olivia) and Jake Smith; and his great grandchildren, Quincy, Alexa, Austin, Carter, Harper and Greyson.
