He was predeceased by: his parents, Pauline (Mullen) and Dale Robert Addicott, Jr.. He is survived by: his children, Dale R. Addicott, III of North Bloomfield, Kevin M. Addicott (Sylvia) of Champion and Tiffany R. Addicott of North Bloomfield, Ohio; his siblings, Paula, Chris, Caroline, Rebecca, Jody, Mark and Jeff; and his grandchildren, Lizzie, Olivia, Daniel, Taylor and Sofia.
