He was predeceased by: his parents, William G. Mayfield and Julia Evans Mayfield Sr.; and his siblings, William G. Mayfield Jr., Marlene Mansfield and Gayle Elston.
He is survived by: his wife Delores Johnson Mayfield; his children, Nicki Geraci (Richard) of New Middletown, Tony Johnson (Tracy) of North Lima and Jack Johnson (Angela) of Sebastian, Fla.; his brothers, Charles Mayfield (Pam) of Mineral Ridge, Gregory Mayfield (Karen) of Vermilion and Duane Mayfield (Sharon) of McDonald; his great grandchild CJ; and his friends, Bobby Wick and Butch Rhodes. He is also survived by seven grandchildren.
In lieu of flowers, the family requests that memorial donations be made to the family to be divided between Ed's favorite charities.
